RN Director of Home Health

Job Type: Full-Time


The Director has 24-hour responsibility and accountability for nursing care within North Metro Medical Center. The position requires knowledge and skill of professional nursing theory and practice, leadership in administration, principles of effective teaching and learning, fiscal management, and effective interpersonal relationships and communication. Knowledge, understanding, acceptance, and support of the philosophy, objectives, and policies of the nursing service and the institution as a whole are essential. Provides leadership and support to the units’ nurse managers and support staff using creative leadership to ensure best outcomes in high quality compassionate patient care. The director works with medical staff and other health care professionals collaboratively to ensure smooth and clinically sound transitions of care regardless of the source of the referral.


  • Coordinates and oversees all direct and indirect patient services provided by Home Health personnel
  • Liaison between health care facilities and home health staff
  • Creates, monitors, and effectively manages departmental budget
  • Monitors and ensures adherence to applicable regulations, policies and procedures
  • Evaluates policies and procedures and assists in development of additional guidelines as required
  • Monitoring, measuring and providing feedback regarding staff performance
  • Documentation, development, and coordination of in-service education
  • Supervise patient visits
  • Coordinates patient records and quality assurance activities
  • Hires, evaluates and terminates personnel
  • Ensures staff are properly oriented, trained and remain current in relevant competencies
  • Timely and regular audit of patient charts to ensure accuracy and relevance
  • Timely and thorough preparation for oversight surveys
  • Establishment and maintenance of effective infection control policies, practices, and training
  • Assures compliance with all local, state, and federal laws regarding licensure and certification of organization personnel.
  • Maintains superior home care standards
  • Assures proper maintenance of clinical records in compliance with local, state, and federal guidelines
  • Monitors and ensures all activities are compliant with CMS regulations; ensures staff are familiar and trained on CMS regulations
  • Assures timely quality assurance audits are regularly conducted on discharge charts
  • Provides effective evaluation of nursing supervisor and staff
  • Assures that audits are conducted on new admission charts for the correct diagnosis, medications, and clinical pathways of care
  • Attends case conferences as needed to ensure proper process are in place to ensure high quality care
  • Facilitates coordination/integration of patient care
  • Other duties as assigned by CNO


  • Integrate the department into the primary function of the Medical Center, including being responsive to changing needs or unusual/urgent events that occur within the Medical Center and participating in cross-organizational activities designed to improve organizational performance, as appropriate.
  • Coordinate and integrate inter- and intra-departmental services.
  • Develop, gain Administrative approval of, and implement policies and procedures that guide and support the provision of departmental services.
  • Recommend a sufficient number of qualified and competent persons to provide departmental services and assist in the Medical Center's recruiting and retention efforts to provide the number of staff recommended.
  • Determine the qualifications and competence of department personnel.
  • Continually assess the needs for and strive to improve the performance of care and quality of the services provided.
  • Maintain, as appropriate, quality control and quality improvement programs.
  • Provide directly or provide access to orientation and continuing education of all personnel in the department.
  • Make recommendations for space and other resources needed by the department as necessary.
  • Participation in the selection of off-site resources needed for services related to the department but not provided by the department.
  • Conduct regular staff meetings to communicate issues regarding the Medical Center's mission, values, operational plans, safety, quality, and other relevant inter- and intra-departmental issues.
  • Provide timely and effective feedback to individuals responsible for coordinating organizational performance improvement activities and, on an individual and departmental level, actively participate in those activities, as appropriate.


  • RN License in good standing within the state of Arkansas
  • Minimum three years Home Health clinical experience
  • Minimum three years of supervisory experience as director or assistant director of Nursing
  • Bachelor’s degree in nursing from an accredited program
  • Master’s Degree in Nursing, Healthcare related Field, or Business preferred


  • Reports to Chief Nursing Officer
  • Directly responsible for exempt supervisory personnel and indirectly responsible for nonexempt professional, technical and nonexempt administrative support personnel reporting to supervisory personnel
