POSITION DESCRIPTION:

The Intake Coordinator works in conjunction with the Community Educator/Clinical Marketers in order to screen potential patients for the program, as well as to assist with community education when needed. The Intake Coordinator, if licensed or licensed eligible, may also assist the Program Therapists with duties such as conducting group therapy as needed. The Intake Coordinator is a member of a multi-disciplinary team and is expected to work with other team members to assist patients in achieving the maximum benefit of treatment.

 

The duties of an Intake Coordinator include:

    • Work closely with the Community Educator in order to convert referrals into admissions
    • Contacting potential patients for screening appointments
    • Screening patients within a 48 hour window
    • Completing all paperwork & putting appropriate information in designated areas for staff in regards to new admits
    • Responsible for pre-certification if required for supplemental insurance
    • If the pt is a NH pt, ensure that the patient’s physician has given an order to the NH allowing the patient to start the program
    • After screening the patient and after obtaining an order from the physician (if NH pt), notify the patient’s family members before the patient begins the program & educate family on the program, the patient’s needs, etc & document
    • Communicate referrals received to the Community Educator
    • Meetings: attend the daily administrative meeting
    • Assist CE when/if no patients are available to be screened during any given day and only as directed by the Program Director
    • If qualified, may assist Program Therapists when/if there is a shortage of staff to run groups and only as directed by the Program Director

 

POSITION REQUIREMENTS:

Work Environment: Works in a hospital environment with visits to patient's place of residence. Universal precautions apply in all environments.

Education: College degree in Social Work preferred. LPN/RN License preferred.

Experience: One year of clinical experience desired. Equivalent experience will be considered.

Physical Demands: Prolonged standing and sitting with some lifting and positioning patients. Assistance from other staff is available.

Other: Participates in continuing education both inside and outside of the hospital setting including hospital committees as needed. Has good leadership skills, communication skills, ability to teach, diplomacy, tact, discretion, and organizational skills. CPR required.
