Mental illness occurs in 10-20 percent of individuals over the age of 55 and is prevalent in 20-30 percent of medically ill older persons. Feelings of sadness, grief, fear, anger, and isolation are treatable symptoms. If left untreated, depression and other illnesses can lead to decreased functioning and loss of independence. The Transitions program addresses the emotional, behavioral, and cognitive needs of adults over the age of 55. Through a combination of medical testing and treatment, therapies, and education we encourage patients to regain as much independence as possible.
A team of specially trained health care professionals provides cohesive, high-quality treatment to each patient. This team consists of:
- Psychiatrists
- Primary Care Physicians
- Occupational Therapists
- Registered Nurses
- Social Workers
- Psychologists
- Case Managers
Referrals are accepted 24 hours a day, seven days a week. A confidential consultation is available at no charge and may be provided in the home, the physician’s office, or the hospital. |
